Formal Network Application and Expense Forms
Formal Networks are those that enter into a covenantal relationship with Presbytery of the Miami Valley for one or more of the following reasons:
- to receive assistance in distributing information through presbytery communication channel.
- to provide services to the presbytery
- to use the Presbytery’s name as an implied endorsement of the ministry
- to request funds from the Presbytery of the Miami Valley or from entities beyond the presbytery
- to acquire the services of consultants to assist a Network’s ministry or mission.
Annual Network Request for Financial Aid (Fillable)
To use, you must download and save the fillable form and then open that saved form. After adding the information, save that filled-in version as a new document with a new name. You can then print the filled-in form or attach it to an email.
